Advanced Directives

Upon arrival it is a good idea to have your Advance Directives (including Living will and Medical Powers of Attorney for Healthcare) with you. If you do not have any of these, please take the time to find out more about these forms by clicking here.  If you have any additional questions concerning these forms please call pastoral care at 678-843-7316.

Insurance

Upon arrival, you may be asked to pay deductibles, co-insurance, private room differences and non-covered amounts prior to service. Saint Joseph’s is a preferred provider meaning we contract with almost all insurance companies. However, if we are not a preferred provider for your plan, you will most likely be subjected to higher co-payment or percentages of your bill. If Saint Joseph’s is not in your provider’s network, you will be responsible for the bill portion not paid for by your insurance.

Pre-certification, Authorization and/or a Physician referral

The patient, guarantor or patient’s physician is responsible for initiating the pre-certification from the insurance company for the particular procedure, test, or imaging. Failure to obtain a pre-certification, authorization, or physician’s referral may result in denial or partial payment for services provided.

Special Information for Medicare Patients

Federal Law requires that Saint Joseph’s Hospital gives all Medicare eligible patients the Medicare Secondary Payor Questionnaire (MSP) when the patient arrives for their surgery, test or procedure. The MSP determines whether or not there is other insurance coverage for the beneficiary that is to be billed before Medicare receives the bill for services rendered. According to federal law, the MSP is to be filled out every time the patient visits the hospital. The MSP can be completed online, over the phone or in person upon arrival.
