Saint Joseph's transplant services

Saint Joseph's Cardiac Transplant Program has been operating since 1987 and has transplanted more than 350 hearts, with survival rates greater than Medicare standards and equal to or higher than nationally accepted standards. Saint Joseph's is one of only two adult heart transplant centers in the state.

Saint Joseph's Cardiac Transplant Program offers comprehensive care to heart failure patients. In partnership with the patients, their families and other providers, and with a commitment to compassion and professional excellence, the cardiac transplant team works to achieve the best possible outcome for every patient. Referral for transplant evaluation can come from physician or patient.

Patient care is managed by a team, which consists of certified transplant coordinators, a social worker, a clinical psychologist, physician assistants, transplant cardiologists and transplant surgeons. Saint Joseph’s is a Medicare-approved transplant facility, and transplant team members are members in good standing of the United Network for Organ Sharing (UNOS).

The referral process
Physician referral for cardiac transplantation is through either direct admission to the hospital or an outpatient screening appointment scheduled with a transplant cardiologist.

Self-referral can occur through an outpatient appointment with a transplant cardiologist.

In addition, a hospital-to-hospital transfer for cardiac transplant evaluation may be initiated after consultation between the referring physician and the transplant medical director.

Each case referred for transplant evaluation is reviewed by the Patient Selection Committee, which determines suitability for cardiac transplantation and the likely clinical outcome.
